• Once you reach there, click at "Yeshasvini" tab or click https://sahakarasindhu.karnataka.gov.in/page/YESHASVINI/en
• Then page having Yeshasvini health insurance scheme details will open.

• Click at "Format for registration of new members for the year 2022-2023" link. 
• Then Yashaswini card application form PDF will open.

• Download this Yashaswini scheme form, take a print-out of the The format should be printed on legal size paper.

About Yeshasvini Health Insurance Scheme

Farmers belonging to economically weaker section are deprived of various healthcare facilities provided by the government. So, Yeshasvini Healthcare Insurance Scheme was launched by Karnataka State Government on 1 June 2003. The Yeashasvini scheme aims to help them in providing comprehensive affordable medical facilities in the state.

Yashaswini scheme is implemented by the Department of Cooperatives and it uses the funds of cooperative societies through Yeshasvini Trust. It provides medical insurance coverage along with different surgical procedures coverage. Individuals who are members of the cooperative society can get benefits of this health insurance scheme.

What is Covered in Yeshasvini Scheme

Yeshasvini Healthcare Insurance Scheme covers expenses of the following surgical procedures:- 

• Cardiothoracic Surgery 
• Cardiac Arrest
• General Surgery
• Paediatric, Orthopaedic Surgery
• Obstetrics
• Gynaecology Surgery Vascular, Neurological, Ophthalmology, Genito-urinary Surgery
• Normal Delivery
• Snake bite
• Dog bite
• Accidents while doing agricultural activities

What is Not Covered in Yashasvini Scheme

The following medical procedures are excluded from Yeshasvini Healthcare Insurance Scheme:- 

• Skin Treatments, Cosmetic Surgery, Grafting
• Kidney transplant
• Heart Transplant Dental Treatment
• Burns
• Regular Check Ups
• Diagnostics Tests
• Nasal Septum Chemotherapy
• Joint Replacement Surgery
• Dialysis
• Autoimmune Diseases

Eligibility Criteria for Yeshasvini Scheme

• Individual below 75 years of age are eligible to apply for this scheme. 
• An applicant should be a member of Karnataka’s cooperative society at least three months prior to the application procedure.
• Spouse, parents and children can also avail the benefits of this scheme.
